having form trouble?

To use our form you need to have macros enabled
in your version of Microsoft Word.

To do this go to TOOLS > MACROS > SECURITY > MEDIUM.
This allows you to decide if you want to run macros each time you open a document that uses them. Macros are snippets of code that allow Word to do tasks automatically.
